Körperliches Training in der Therapie von Herzerkrankungen

Zusammenfassung

Ein gezieltes körperliches Training als eine ergänzende Option zur medikamentösen und interventionellen Therapie hat in die kardiologischen Leitlinien Einzug erhalten. So zeigen Trainingsprogramme bei kardiovaskulären Risikofaktoren wie der arteriellen Hypertonie, beim Diabetes mellitus Typ 2 und bei Dyslipoproteinämie ebenso positive Effekte wie bei der koronaren Herzerkrankung, nach akutem Myokardinfarkt, bei Herzinsuffizienz mit erhaltener oder eingeschränkter linksventrikulärer Pumpfunktion, bei Vorhofflimmern, nach kathethergestützem Aortenklappenersatz (TAVI), nach Implantation von implantierbaren Kardioverter-Defibrillatoren (ICD) oder linksventrikulären Unterstützungssystemen (VAD). Diese Trainingsprogramme müssen auf Krankheitsentität, -stadium, Komorbiditäten, Alter des Patienten, Medikation und Belastbarkeit individuell abgestimmt werden. Hierzu ist in Ergänzung zur kardiologischen Untersuchung vor Beginn eines körperlichen Trainings eine maximale Ergometrie durchzuführen. Optimalerweise wird diese ergänzt durch eine Spirometrie zur Bestimmung der maximalen Sauerstoffaufnahme, anhand derer die Intensität des Trainings durch Pulsvorgaben festgelegt und mittels einer Pulsuhr überprüft werden kann.
