Gastroösophageale Refluxerkrankung (GERD) aus Sicht der westlichen Medizin und aus der Perspektive der Traditionellen Chinesischen Medizin (TCM)

Zusammenfassung

Die gastroösophageale Refluxerkrankung (GERD) ist eine häufige Volkskrankheit, die ca. 40 % der Weltbevölkerung betrifft. Die GERD beschränkt die Lebensqualität (QoL), ist häufig mit einer Dauerverordnung und potenziellen Neben‑/Wechselwirkungen von Protonenpumpenhemmern (PPI) verbunden und weist einen relevanten gesundheitsökonomischen Aspekt auf. Patienten fragen immer wieder nach Alternativen zu PPI, westlich gibt es keine adäquate symptomlindernde Option.
Die Traditionelle Chinesische Medizin (TCM) mit ihrer mehr als 2000-jährigen Erfahrung und Bewährung bietet sich als wirksame komplementäre Option an. Nach der aktuellen Studienlage ist bei GERD eine additive Akupunktur zur westlichen Therapie mit PPI zu befürworten. Akupunktur in Kombination mit westlicher Standardtherapie führt zu einer besseren Symptomreduktion und kann v. a. bei PPI-refraktären Patienten eine gute Option sein. Akupunktur und chinesische Phytotherapie zeigen bei der GERD-Behandlung keine schweren unerwünschten Nebenwirkungen. Chinesische Phytotherapie führt zu einer Symptomreduktion bei GERD und weist einen besseren Langzeiteffekt auf als PPI. Die Evidenz für TCM-Phytotherapie bei GERD ist jedoch gering. Die Studien weisen größtenteils ein schlechtes Studiendesign und geringe Fallzahlen auf. Zudem arbeitet die TCM mit individuellen Therapiekonzepten entsprechend der chinesischen Diagnose. Bei dieser Heterogenität der Patientengruppen ist die TCM nicht für westliche evidenzbasierte Medizin (EBM) geeignet. Eine Option wären hier westliche randomisierte kontrollierte Studien (RCT) nach dem Vorbild der in China durchgeführten „TCM-Syndrom-Studien“, die die Patientengruppen entsprechend der gleichen chinesischen Diagnose randomisieren.
Nach der untersuchten Studienlage bleibt TCM bei GERD eine erfahrungsbasierte Medizin. Bei geringem Nebenwirkungsprofil und positiven Trends in den Studien kann jedoch den PPI-refraktären GERD-Patienten und den Patienten mit nicht erosiver Refluxkrankheit (NERD) mit psychischer Komorbidität TCM als wirksame Option empfohlen werden.
