Left main coronary artery (LMCA) atresia is an extremely rare anomaly. LMCA atresia should be differentiated angiographically from the single coronary artery. The flow direction in the coronary artery is determinant in the differential diagnosis. Intercoronary communication is also an extremely rare condition characterized by an opened circulation between two coronary arteries. The association of intercoronary communication with LMCA atresia can further complicate the diagnosis.
