Wann und wie revaskularisieren?

Zusammenfassung

Chronische Koronarverschlüsse („chronic total occlusions“, CTO) liegen bei rund 20 % der PCI (perkutane Koronarintervention)-Patienten und bei 50 % der Patienten nach Bypass-Operation vor. Infolge des technischen Fortschritts mit retrograden Rekanalisationsmöglichkeiten besteht in spezialisierten Zentren, unabhängig von der Verschlussanatomie, ein über 85%iger Rekanalisationserfolg. Die retrograden Methoden haben einen hohen technischen Anspruch und sollten nach einem Konsensusdokument des EuroCTO-Clubs nur von Interventionskardiologen mit ausreichender Erfahrung in den antegraden Techniken (>300 antegrade CTO-Interventionen, >50/Jahr) nach zusätzlicher Schulung (jeweils 25 retrograde Fälle als Zweit- und Erstuntersucher) selbständig durchgeführt werden. Der hohe technische und zeitliche Aufwand ist allerdings aktuell nur bei einem erkennbaren klinischen Nutzen für den Patienten gerechtfertigt. Dieser technische Fortschritt und die immer deutlicheren Hinweise, dass bei Patienten mit koronarer Mehrgefäßerkrankung eine möglichst komplette Revaskularisation angestrebt werden sollte, haben in den letzten Jahren zu einem zunehmenden Interesse der Interventionskardiologen an der Rekanalisation von CTO geführt.
