Arthroskopische Rotatorenmanschettenchirurgie

Zusammenfassung

Der vorliegende Beitrag präsentiert eine Übersicht von etablierten sowie gegenwärtigen Entwicklungen und Aspekten der Rotatorenmanschettenchirurgie. Hierzu werden erreichte Meilensteine, aber auch jüngere technisch-rekonstruktive Ansätze mit klinisch-radiologischen Resultaten gegenübergestellt und deren jeweiliger Stellenwert beleuchtet. Trotz technischer und biomechanischer Weiterentwicklungen von Rekonstruktionstechniken der Rotatorenmanschette konnte die Rate von Redefekten bzw. Rerupturen nicht eliminiert werden. Aufgrund der zunehmend verstandenen Bedeutung der biologischen Charakteristika erfolgreicher Sehneneinheilung wird der aktuelle wissenschaftliche Stand der experimentellen Anwendung von Wachstumsfaktoren, der klinischen Anwendung von Platelet Rich Plasma, sowie der Rotatorenmanschettenaugmentationstechniken (mittels allogenem und autologem Gewebe) dargestellt. Zusammengefasst gibt es klare biomechanische Fortschritte und vielversprechende Ansätze zur biologischen Augmentierung, wobei Letztere bis dato jedoch noch keinen Transfer in die klinische Anwendungsroutine gefunden haben.
