A 52-year-old female patient with rheumatic mitral valve stenosis and chronic atrial fibrillation underwent mitral valve replacement consisting of a bileaflet mechanical valve, left atrial maze procedure, and resection of the left atrial appendage. Postoperatively, the patient converted to normal sinus rhythm. However, because of the mechanical mitral valve she was discharged on warfarin anticoagulation. Unfortunately, her anticoagulation treatment was not followed up well as an outpatient, and she was found to have subtherapeutic international normalized ratio levels on several occasions. …
