Acute respiratory distress syndrome (ARDS) was first described 1967 [
1]. Mortality is high, around 30–40 %, with only slow decrease over the years. How can one then say that there are physiologic advances that have improved pulmonary treatment? One reason is that patients with increasing disease severity and increasing age are more frequently admitted to intensive care units and there receive the diagnosis of ARDS.
