4. Nuklearmedizinische Diagnostik

Zusammenfassung

Nuklearmedizinische Verfahren nehmen seit Jahren einen wichtigen Stellenwert in der Abklärung kardiologischer Krankheitsbilder ein und sind daher auch für den Herzchirurgen von Bedeutung. Nuklearkardiologische Untersuchungstechniken sind wegen ihrer diagnostischen und prognostischen Aussagekraft, Zuverlässigkeit und Reproduzierbarkeit besonders in der Abklärung der koronaren Herzerkrankung (KHK) ein wichtiger diagnostischer Parameter. Konsequenterweise wurden die nuklearmedizinischen Verfahren in die Empfehlungen und Richtlinien zur Abklärung der KHK der amerikanischen (AHA/JACC) und europäischen (ESC) kardiologischen Fachgesellschaften aufgenommen (Marcassa et al. 2008; Montalescot et al. 2013). In dieser Übersicht findet sich eine klinisch orientierte Zusammenstellung der beiden bedeutendsten Einsatzgebiete nuklearkardiologischer Verfahren, der Myokardperfusionsszintigraphie (MPS) und myokardialen Vitalitätsdiagnostik. Für jede Indikationsgruppe werden die Methodik, die Befundung, die Aussagekraft sowie die Limitationen des entsprechenden nuklearkardiologischen Verfahrens dargestellt.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
