Nierenarterienstenose im Kindesalter

Darstellung eines Fallbeispiels mit interventioneller Therapie und Übersicht über die Literatur

Zusammenfassung

Hintergrund.

Der renovaskuläre Hypertonus ist im Kindesalter zwar selten, aber dennoch nach der Säuglingsperiode die häufigste kausal behandelbare Form des Hypertonus im Kindes- und Jugendalter. Die Nierenarterienstenose stellt hierbei eine heterogene Gruppe an Erkrankungen dar, die sowohl intrinsische Gefäßveränderungen als auch extrinsisch erzeugte Stenosen in sich vereint.

Fallbericht.

Wir beschreiben in dieser Kasuistik die erfolgreiche Therapie einer signifikanten einseitigen Nierenarterienstenose bei einem 12 Jahre alten Mädchen, bei dem mit Hilfe eines speziellen Dilatationsballons (so genannte Cutting-balloon-Technik) eine mit den üblichen interventionellen Methoden therapierefraktäre Gefäßstenose dilatiert werden konnte.

Resümee.

Im Weiteren werden die besonderen diagnostischen und therapeutischen Möglichkeiten bei dieser Krankheitsgruppe aufgezeigt und ein Überblick über die vorliegende Literatur gegeben.

Manifestiert sich ein Typ-1-Diabetes bei Kindern, ist das ein Notfall – ebenso wie eine diabetische Ketoazidose. Die Grundsäulen der Therapie bestehen aus Rehydratation, Insulin und Kaliumgabe. Insulin ist das Medikament der Wahl zur Behandlung der Ketoazidose.

Frühe Hypertonie erhöht späteres kardiovaskuläres Risiko

Wie wichtig es ist, pädiatrische Patienten auf Bluthochdruck zu screenen, zeigt eine kanadische Studie: Hypertone Druckwerte in Kindheit und Jugend steigern das Risiko für spätere kardiovaskuläre Komplikationen.
