Mitralinsuffizienz bei Herzinsuffizienz

Zusammenfassung

Die optimale Therapie der herzinsuffizienzbegleitenden Mitralinsuffizienz bleibt umstritten. Ein fehlender Konsens zwischen Kardiologen und Kardiochirurgen – aufgrund eingeschränkter Studien- und heterogener Datenlage – hat in der Vergangenheit zu vage definierten Leitlinien und unterschiedlicher Handhabung in der klinischen Routine geführt. Fortschritte im operativen Management mit exzellenten Ergebnissen einzelner erfahrener Zentren legen allerdings nahe, dass eine Vielzahl von Patienten von einer chirurgischen Therapie zu profitieren scheint. Die Evaluation hinsichtlich des Nutzens einer chirurgischen Therapie sollte hierbei für jeden Patienten individuell vorgenommen werden und erfordert aufgrund der komplexen Pathophysiologie und der anspruchsvollen Diagnostik einen interdisziplinären Ansatz („Heart-Team“).
