Diagnostik potenziell maligner Mundschleimhautveränderungen in der Zahnarztpraxis

Zusammenfassung

Auch wenn orale Karzinome und deren Vorstufen klinisch oft nicht erkennbar sind, bestehen bei der Mehrzahl der Patienten vorher visuell erkennbare potenziell maligne Veränderungen (PMV). In Mitteleuropa sind dies v. a. Leukoplakie und oraler Lichen planus, seltener Erythroplakie. Liegt eine PMV vor, muss das lebenslang erhöhte Krebsrisiko durch die Untersuchung auf eine orale Schleimhautdysplasie abgeschätzt werden. Dies erfordert eine Inzisionsbiopsie als diagnostischen Goldstandard, auch zum Ausschluss der Malignität. Die Auswahl der repräsentativen Biopsiestelle ist oft schwierig. Umfang und Zeitpunkt invasiver Diagnostik richten sich nach Risikofaktoren (v. a. Tabak- und Alkoholkonsum) sowie systematischer Untersuchung der Mundschleimhaut. Wichtige Ergänzung sind die Bürstenbiopsie einzelner Läsionen sowie intravitale Färbung, Chemolumineszenz und Autofluoreszenz für die gesamte Mundhöhle. Letztere können die Auswahl der Biopsiestelle und die Tumorerkennung minimalinvasiv erleichtern.
