15. Biologischer und mechanischer Herzersatz

Zusammenfassung

Die Herztransplantation als biologischer Herzersatz ist heute immer noch die effektivste Therapie der terminalen Herzinsuffizienz. Die Auswahl des optimalen Empfängers, aber auch die Spenderselektion werden ausführlich dargestellt, ebenso die verschiedenen Techniken der Organentnahme im Spender als auch die orthotope Implantation des Herzens im Empfänger. Die heute nur noch kasuistisch angewandte heterotope Herztransplantation wird kurz vorgestellt. Perioperative Komplikationen, aber auch der postoperative Verlauf mit der nach Transplantation obligaten Immunsuppression, der Physiologie des transplantierten Herzens bis hin zur Endomyocardbiopsie und Retransplantation, werden beleuchtet. Die Geschichte des Kunstherzens (total artificial heart – mechanischer Herzersatz), seine Indikation, die Implantation und die weiteren Entwicklungen auf diesem innovativen Gebiet werden kurz dargestellt.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
