Technik und Grenzen arthroskopischer Versorgung von Glenoid- und Skapulafrakturen

Zusammenfassung

Durch die Weiterentwicklung der arthroskopischen Techniken hat die Versorgung von Frakturen des Glenoidrands in den letzten Jahren neue Möglichkeiten erfahren. Potenzielle Vorteile ergeben sich besonders durch die Möglichkeit der Diagnostik und Therapie von Begleitverletzungen sowie eines minimalinvasiveren Zugangs unter Schonung des M. subscapularis. Beschreibungen der Operationstechniken und kleine Fallserien zeigten bisher vielversprechende Ergebnisse. Auch die Versorgung von Skapulafrakturen mit Gelenkbeteiligung scheint potenziell arthroskopisch bzw. arthroskopisch assistiert umsetzbar zu sein. Hier kann aber ebenfalls aufgrund kleiner Fallzahlen mit niedrigem Evidenzgrad noch keine abschließende Aussage getroffen werden. Höhergradige mehrfragmentäre Kombinationsfrakturen von Skapula und Glenoid bleiben weiterhin dem offenen Vorgehen vorbehalten.
