12. Onkologische Krankheiten

Zusammenfassung

Krebserkrankungen stellen führende Todesursachen in den meisten Ländern dar. Fortschritte der modernen Medizin haben in vielen Fällen aber ermöglicht, mit Krebs als einer „chronischen Krankheit“ zu leben. Krebserkrankungen sind mit signifikant erhöhten Prävalenzraten koexistenter affektiver und Stress-bezogener Störungen assoziiert. Komorbide psychische Störungen führen zu einem erhöhten Krankheits-inhärenten Distress, zu einer beeinträchtigten Lebensqualität und zu einer nachteiligen psychosozialen Integration. Die Frage aber, inwieweit affektive und Stress-bezogene Störungen die Entwicklung zu einer definierten Krebserkrankung im Sinne eines erhöhten Inzidenzrisikos beeinflussen, wird höchst kontrovers diskutiert, ebenso auch ein potentieller Einfluss auf die biologische Krankheitsdynamik, die Morbidität und Mortalität. In einer ätiopathogenetischen Perspektive existieren erste Modellvorstellungen, wie psychologische/ psychosoziale Stressfaktoren auf einzelne Abschnitte der Kanzerogenese, auf unterschiedliche onkobiologische Mechanismen Einfluss nehmen und umgekehrt in einer Rückmeldung an zentralnervöse Prozesse zu einer erhöhten affektiven und kognitiven Vulnerabilität beitragen könnten. Onkologische Behandlungsmodalitäten (Chemo-, Hormon-, Immun-, Radiotherapie, Chirurgie) beeinflussen eigenständig das Risiko koexistenter affektiver, Stress-bezogener und kognitiver Störungen. Studien zur Psychotherapie und Pharmakotherapie koexistenter affektiver und Stress-bezogener Symptome bei onkologischen Krankheiten werden referiert.
