Zusammenfassung
Die Urethritis posterior ist eine seltene, meist selbstlimitierende, gutartige Erkrankung die bei präpubertären und pubertären Jungen auftritt. Die typischen Symptome sind eine meist terminale Makrohämaturie nichtglomerulären Ursprungs und/oder postmiktionelle Blutspuren am Meatus oder in der Unterhose. Eine Urethrozystoskopie zur Diagnosestellung ist, außer bei Tumorverdacht, nicht notwendig, da durch die Endoskopie iatrogene Urethrastenosen erzeugt werden können. Umso wichtiger ist der Ausschluss einer Harnröhrenstenose mittels Restharn- und Harnflussmessung.
Die Urethritis posterior ist eine Ausschlussdiagnose mit einer sehr guten Prognose und einer Spontanheilungsrate von über 90% in 2 Jahren.
Bei Kindern mit Miktionsauffälligkeiten, wie z.B. einer „Bladder Bowel Dysfunction“ (BBD) ist eine urotherapeutische Behandlung z.B. im Sinne eines Biofeedback-gestützten Miktionstrainings erfolgsversprechend.
