Zusammenfassung

Die Insulintherapie bei Patienten mit Diabetes mellitus Typ 1 und insulinbehandeltem Diabetes mellitus Typ 2 im höheren Alter stellt eine besondere Herausforderung dar, weil es sich um ein spezielles und sehr heterogenes Patientenkollektiv handelt. Während bei Typ-1-Diabetes aufgrund des absoluten Insulindefizits eine lebenslange Insulintherapie erforderlich ist, sollte bei Älteren mit Typ-2-Diabetes geprüft werden, ob ein insulinfreies Therapieregime möglich ist. Insulinregime sollten so einfach wie möglich sein, um eine adäquate Umsetzbarkeit im Alltag Älterer zu gewährleisten. Mit fortschreitendem Lebensalter kommt es oft zu zunehmenden Komorbiditäten, die eine Änderung der Therapieziele erforderlich machen können. Besonderes Augenmerk sollte auf eine evtl. entstehende kognitive Dysfunktion gerichtet werden. Schwere Hypoglykämien sind im höheren Alter unbedingt zu vermeiden. Die Therapieziele bei insulinabhängigen älteren Diabetespatienten müssen stets individuell an die klinische Gesamtsituation, die Wünsche und Bedürfnisse des Einzelnen und seine Ressourcen angepasst werden.
