Update zur Behandlung der Aortenklappenstenose bei symptomatischen und asymptomatischen Patienten

Zusammenfassung

Die Aortenklappenstenose gehört in den Industrienationen zu den häufigsten behandlungsbedürftigen Vitien. Das Auftreten von Symptomen geht mit einer deutlich erhöhten Mortalität einher, sodass bei einer symptomatischen, hochgradigen Aortenklappenstenose eine eindeutige Behandlungsindikation besteht. Die Datenlage zur optimalen Behandlung der asymptomatischen Aortenklappenstenose ist hingegen weniger eindeutig. Studien aus dem herzchirurgischen Bereich deuten darauf hin, dass ein frühzeitiger operativer Klappenersatz einem konservativen Vorgehen überlegen ist. Aus diesem Grund werden die Ergebnisse weiterer randomisierter Studien mit großem Interesse erwartet. In diesem Jahr wurden zudem zahlreiche Langzeitergebnisse randomisierter Vergleiche der beiden verfügbaren Therapieoptionen (chirurgischer vs. kathetergestützter Klappenersatz) veröffentlicht, die nun in die Entscheidung des Heart-Teams zum individuell besten Behandlungsverfahren einfließen.
